- DVS Request Form
- Your original passport, valid at least six(6) months upon entering Cambodia
- Visa pages available for Visa sticker at least two(2) pages
- No loose pages/attachments
- One (01) Visa application forms to be fully completed and signed by the applicant
- One (01) Attached 2x2 Passport picture recent photo at least six(6) months
- Letter of invitation from the Company/ Organization in Cambodia/abroad for the business trip (one copy) (For coordination, please insert contact person, email address and direct number to the cover letter)

- Official Letter issued by the Department of State/Organization - 3 copies Purpose of trip must be detailed information like WERE the Officer heading to Cambodia, WHOM they report and designation, DATE of entry and departure in Cambodia, DATE of departure to the United States, MEETING with whom and designation (kindly insert the Names and designation) and purpose of meeting.
