- DVS Request Form
- Original, signed U. S. Passport Please submit the actual passport and a copy of the main passport page
- Two(2) Visa Application Form. Completed visa forms: one (1) original and one (1) copy
- One(1) Recent Passport-type photograph (2″ x 2″) recent high definition personal passport photos on white background stapled or glued to each visa application
- Yellow Fever (YF) Vaccine A copy of the YF immunization card/record. Exemption letter/statement from the primary physician/medical provider
- An invitation letter from a Gabonese based company or business acquaintance/affiliate conducting business in Gabon
