Tourist Visa Requirements

• DVS Request Form
• Current passport Signature field must be signed and passport must have at least 6 months validity from day of return.
• Regular Visa Application Form. Fully completed MUST be Signed and Dated
• Or
• Expedited Visa Application Form. Fully completed MUST be Signed and Dated
• The application must be typed/completed in Black Inc
• Applicants must answer all questions on the form leaving no question unanswered
• Full Name on Application MUST match the Name in the Passport
• Two (2) Recent Passport-type photograph (2″ x 2″) recent high definition personal passport photos on white background. NOT PHOTOCOPIES
• Provide two (2) Contact Persons/places in Ghana /Indicate if Hotel/Guest House etc.
  (Applicant MUST indicate the full name, complete address, and tel. no/s)
• Attach helpful supporting document/s relating to your purpose of the journey
• Photocopy of applicant’s bank statement
• Photocopy of proof of hotel reservations Provide possible sightseeing places you will be
  visiting in Ghana
• Provide complete Residential Addresses and Tel. No/s in the USA where required.
• For Minor Applicants (Under 18 Years of Age) the following documents must be included with the child's application to Ghana:
  • A copy of minor's birth certificate showing name of both parents;
  • A copy of legal documents for custody (if applicable);
  • A copy both parents' valid photo IDs;
  • A legalized letter of consent.
