Your original passport, with at least 6 months of validity
One (01) Visa application forms to be fully completed and signed by the applicant
One (01) Attached 2x2 Passport picture (recent photo at least six(6) months)
All application for minor(s) has to be accompanied by 1. Birth certificate (copy) 2. Notarized consent letter signed by the parent(s) who is not traveling with the kid(s) or custodial documents
