One (01) Visa application forms to be fully completed and signed by the applicant
One recent passport-size photograph. (Photo must be Glued on the top right of an Application Form)

For Diplomatic One (01) Diplomatic Visa application forms to be fully completed and signed by the applicant
For Official One (01) OfficialVisa application forms to be fully completed and signed by the applicant
One (01) Attached 2x2 Passport picture (recent photo at least six(6) months (Photo must be Glued on the top right of an Application Form)
A note verbale from the State Department/diplomatic mission/ international organization or equivalent offices with a request for a diplomatic visa, stating clearly the purpose and period of the visit
