SECOND PASSPORT

Issued at the discretion of U.S. Department of State to frequent travelers that do not have time to acquire visas between trips; or for security reasons.

REQUIREMENTS

DVS Visa Services Submission Sheet. Outside the sealed envelope

Application form DS-11  fully completed and printed. (This form must be completed online at the Department of State web site and will print with a special bar code.) Please do not sign the application until you take the oath administered by the passport acceptance agency.

A valid and signed US passport.

Signed by APPLICANT, substantiating the need for a second passport

1 Recent Passport-type photograph (2″ x 2″) in color, front view with white background.

A copy of the airline ticket or travel itinerary showing the departure date within three days (if no visa is required) or within ten days if a visa is required of the date of application.

Under the age of 16: $140.00.

A self-addressed return mail-label (FedEx, UPS, USPS) to be used to convey the processed document(s) back to the applicant.

All the above requirements should be taken to a main US Post office or a Circuit court where your application will be executed in front of the clerk. The clerk will enclose the information in a sealed envelope. Once it has been sealed it cannot be reopened.
