Business Visa Requirements
- DVS Request Form
- Original, signed U. S. Passport with at least 6 months of remaining validity beyond stay in Zambia, Your passport must have a minimum of 2 blank pages marked with the words "VISA" or "ENTRIES"
- Two (2) Visa Application Form. Please download, print, complete and sign the Visa application. must be submitted in duplicate
- Photocopy of passport page containing personal details
- 2 Recent Passport-type photograph (2″ x 2″) in color
- White background
- Please SIGN the back of each photograph and staple one photo to each application form the Upper Right corner
- Signed Letter of Invitation from their host in Zambia
- Signed Cover letter on Official letterhead from the Sponsoring Organization in the United States
- Attach a copy of the flight itinerary and/or return airline ticket
- HEALTH REQUIREMENTS: Anti-Malaria vaccination is strongly recommended.
